The integrated team of social workers, case managers and discharge planners ensure that patients, who are medically stable, and/or their families have the knowledge and resources necessary for a safe discharge. All social workers have a Masters degree and provide counseling and support to patients and families throughout the hospital stay. Additionally, they are liaisons between patients and long-term care and other alternative facilities, as well as linking patients to appropriate state and federal programs when financial problems arise. Discharge planners are registered nurses who assess the progress of each patient, collaborate with attending physicians and other health care providers, and arrange for home care services and equipment, as needed. Case managers, also registered nurses, serve as liaisons between health care team members, patients, families and the JFK Medical Center staff. An on-site Medicaid worker is available by appointment to assist in the Medicaid application process. In addition, the Coordinated Health Department provides office space for a representative of the Visiting Nurses Association of Central New Jersey -- the only Medicare-certified home care agency in Middlesex County.
