Health Information Services maintains the medical records of inpatients discharged from the hospital. Copies of medical records can be requested with appropriate patient authorization. A state-regulated fee may be charged for copies unless they are needed for continuing patient care.

For copies of birth certificates, please contact the Bureau of Vital Statistics, Edison office, at 732-248-7293.
