Medical Records is located at the JFK-Muhlenberg Campus. To access your medical records from Muhlenberg, a patient or patient's legally authorized representatives can request, in writing, copies of the medical records. Requests should be sent to:

One copy of the medical record from an individual admission shall be provided to the patient or the patient's legally authorized representative within 30 days of request.
