The Home Care Program at the JFK-Muhlenberg Campus is an expansion of the quality healthcare services available at the Medical Center with one special difference…the care is provided in the comfort and convenience of the patient’s home. With the support of JFK-Muhlenberg Campus healthcare providers, patients can receive quality care in the comfort of their own home rather than at a hospital or long-term care facility. The program offers a breadth of health and home care services to meet the diverse needs of our patients.
We provide a coordinated system of comprehensive, multidisciplinary healthcare services in a patient’s home to promote, maintain and/or restore health, and minimize the effects of illness and disability. Upon the initial home visit, our skilled nurses -- working closely with a team of healthcare professionals -- assess the patient’s home environment, physical and clinical needs; as well as coordinate the patient’s plan of care with his/her physician to ensure the best outcome. Additionally, home health aides are available to make the patient’s recuperation at home a comfortable experience, performing a variety of in-home services including personal care, daily light housekeeping, grocery shopping, among other tasks.
Home health care services may be provided as an alternative to a hospital stay; however, a physician’s order is needed to obtain a service. When hospitalized, discharge planners will make arrangements for necessary follow-up care upon release. A referral can be made from family member, physician or other healthcare professional. Services are provided by an interdisciplinary team of trained professionals.

- Assessment and Teaching of Disease Process Management
Physical, Occupational, and Speech Therapists Provide:
- Rehabilitation from stroke, fractures and other disabilities through development of an exercise program.
- Instruction on the use of assistive devices (ex: walkers).
- Homemaking Skills.
Home Health Aides Provide Assistance In:
- Bathing
- Dressing
- Feeding
- Meal Preparation
- Light Housekeeping
Social Workers Provide:
- Counseling
- Referrals to Community Resources
Registered Dietitians Provide:
- Outline of Nutritional Program
- Special Diet Instructions
- Instructions about Meal Planning and Food Preparation
An on-call nurse is available 24 hours a day – 7 days a week.
Additionally, the Home Care Program at the JFK-Muhlenberg Campus provides telehealth monitoring.
The Home Care Department is licensed by the New Jersey Department of Health, accredited by the Joint Commission, and is certified to provide home health services by Medicare and Medicaid. Services are provided in accordance with physician orders and Medicare, Medicaid or commercial insurance guidelines. Insurance carriers will be billed directly.
